• Facility Application - To be filled out by a representative of the facility requesting visits.
  
  
• Log Sheet - To be used as a sign in sheet every time a volunteer visits a facility; should be available in designated location in the facility.
  
  
• Photo Consent Form - Must be filled out in its entirety whenever a resident living in a facility is photographed.
